Final forms can be accessed from our homepage at the top right by clicking on “Popular Links” or under the Parents and Students menu by clicking the Forms and Enrollment option. The direct address is the link here. If you'd like, you can watch a video that walks through the process below, just click here.
First, click on the "Parent" link at the left, and then you'll need to log in with your Final Forms username and password. If you can't remember it from this past year, you can use the "Forgot password" or "forgot your email" links on the right. If you didn't get a chance to fill out your forms this past year and haven't registered yet, you can click the "Never logged in" link at the bottom. It will ask for your email and send you the steps to get started. If you can't get logged in, please contact me, Mr. Keehn at the cg_keehn@cg.noacsc.org or (419) 659-5090.
All students and staff as well as any parent whose account is connected to a Gmail address can now use the "Login with Google" button at the top of the login page. This makes it such that you do not have to remember a separate password for Final Forms. You can also continue to sign in with the email and password you created.
Once you are logged in you should see your students. Note, if you are logging in before the new year has switched over (mid-June), you'll want to click on the big "Switch to 2x-2x registration" button at the top and click "Ok, let's get started." If after the switchover date you are already where you need to be for next year.
Click on the "incomplete forms" button next to one of your students to get started for the new year. Here you can select the sports that they plan to participate in - even if they are just trying out or aren't sure, select the sport to make sure that you are presented with all of the required forms for Athletics. Click "Update Sports" to continue.
Next, you will see all the forms that need to be filled out. You will see different forms based on what grade your student is in and if they are competing in JH or HS sports. If a form is in red, the parent has to fill it out, if it is in yellow, the parent has filled it out but not the student, and if it is in green it is complete. If you entered all of your information this past school year, all you need to do is verify the info that is in there and "sign" at the bottom by typing your name. It's very quick. If you didn't fill them out this past year, while it looks like a lot, many of the forms are very short and it doesn't take more than 15-20 minutes to fill them all out. If you have more than one student, much of the information copies to their forms as well so you don't have to retype it for each one.
The last form that will be listed for athletes is the "OHSAA PPE Physical (for Physicians)" form. After you complete the Health History & Medical Profile along with the OHSAA Authorization, you can go to this last one and use the "Click to Print - OHSAA PPE Physical Form" to print out the form that your Physician will have to sign for the Physical. All of the answers that, in the past, had been filled out by hand are pre-printed on the form for you. If you do not have access to a printer, please contact Mrs. Pingle at cg_pingle@cg.noacsc.org and she can assist you with getting one printed off. Like in the past, this form must be on file with the school before your student can participate in any sports.
If your student has to sign any forms, they will get an email at their school email address that prompts them to log in and do that. Once everything is signed, you'll be ready for the new year. Once again, if you need any assistance use the info at the top of this page to contact Mr. Keehn.
Below you can view the process for setting up a Student or Parent ProgressBook account. To get started you'll need a registration key. HS/MS Students will have the code on their schedule, HS/MS Parents will receive a note that includes the code along with the emergency forms the first day of school; Elementary Parents will receive the code during Open House, or it will be sent home the first day with your student if you are unable to attend. If you don't have the code or are new to the district, you can get the code from your teacher or Mr. Keehn. When you have that code, here are the steps you will need to follow to register:
The process to sign up is fairly simple. Navigate to the site here.
Our Chromebooks use an app called PaperCut Mobility Print to connect and print to our printers. The first time you try to print you’ll have to click on the “see more” option from the printer drop down box to see all of the CG printers. If you still don’t see them, please let Mr. Keehn know.
If part of your page is getting cut off when you print you may have adjusted some print settings for another print job and forgot to change them back. For example, when printing Desmos projects in math classes you sometimes have to zoom in to get it to fit on the page correctly. To change it back follow these steps.
Click on print as you normally would.
Select the printer you want to send the job to at the top right of the print dialog.
At the bottom right of the dialog click on "More settings."
You should now see an option called "Scale" with a drop down box next to it.
Change the selection to "Default"
Your print job should now print correctly.
In order to make pages load faster your Chromebook will store parts of the page locally on your computer so it doesn't have to download them every time. This is called caching.
From time to time items in the cache will need updated, become corrupted, or have other issues and will cause errors with your pages when they load.
To correct these issues we may have to clear the cache. To do so, follow these steps:
Press Ctrl+Shift+Backspace on your keyboard. (Or click the three dots at the top right of the screen, go to "More tools," and click on "Clear browsing data.")
Click on the "Advanced" tab at the top of the box that appears.
At the top select "All time" from the Time Range drop down box.
Check the two boxes named "Cookies and other site data" and "Cached images and files." You can uncheck all of the others.
Click on "Clear data."
Keeping your Chromebook up to date is one of the easiest ways to make sure you don't have issues with pages not loading, apps not working, etc. If your Chromebook is behind on updates you may see all kinds of strange errors and things not working. The device will eventually update itself, but to force it to load the latest update use the following steps.
At the bottom right, click on the time and then the Settings gear at the top of the menu.
At the bottom left of the page that comes up, select About ChromeOS.
Under "Google ChromeOS," find which version of the Chrome operating system your Chromebook uses.
Select Check for updates.
If your Chromebook finds a software update, it will start to download automatically.
Most of the time the answer is yes. If the content of your document is missing but you still have the file you can usually get the content back. Google has a built in feature called Version History.
In Docs, Slides, or Sheets click on File at the top left, then hover over "Version History."
Click on "See version history."
On the right side of the page you will now see names of people that edited the document, usually just you, along with dates. You can click on the black arrow next to a date to expand that version and then click on individual times to see the version of the document at that point in time.
Click on the three dots on the right to restore a version, name a version, or make a copy of that version of the document. Typically you will select restore this version and it will bring back your content.
If your file is missing completely it may have been accidentally deleted. To try and find it open up Google Drive in your browser. Click on the Trash menu item on the left side.
Find your file on the right side, right click on it, and click on "Restore" and your file will be put back where you originally had it.
If you don't see your file in there, please contact me and I can try and see if there is anything else I can do to find the file.
On certain models of our Chromebooks there is a design flaw that causes the cable that connects the touch pad inside your Chromebook to become disconnected.
I can open it up and get it reattached, just stop by my office.
